Dutch photographer Koen Hauser is famous for his unusual and creepy photography style.

Effective April 21, 2009, CMS has stated that if rated ages do not conform to their standards for “acceptable proof” the allocations will be priced using the actual age and CMS will not consider re-pricing the case even using new or corrected information.
Per CMS acceptable proof of a Rated Age must include the following:

be independent
on letterhead of insurance carrier or settlement broker

The submitter must then state clearly that “all rated ages obtained on the claimant have been included.”

Walmart and Sam’s Club report that they will offer electronic medical records software by eClinicalWorks for the discount price of $25,000.  What?! Who thought Walmart or Sam’s selling EMR was a good idea.  Don’t get me wrong I love Walmart.  It is where I do all my food shopping as well as sundries and supplies.  It’s also a great place for flip flops, but EMR?  I don’t get it.
Why would Walmart enter the EMR market?  Sam’s Club seems a bit better suited as they do cater to the business crowd, …
