Outpatient surgical errors may go unreported

Surgical errors that occur at outpatient or ambulatory surgical centers may be going unreported because of a flaw in the Indiana rule for reporting such mistakes. Under the current rule, healthcare providers are required to report errors as they are discovered at their facility. However, in the outpatient model patients are unlikely to return to the same facility to receive care for problems caused by medical error. Doctors say that such reporting errors could grow as more procedures are moved from hospitals to free-standing facilities.  Jennifer L. Boen, Fort Wayne News-Sentinel
